Prevenção das Crises Álgidas na Anemia Falciforme pelo
Inibidor da Enzima Conversora da Angiotensina

ABSTRACT
According
to a new hypothesis the sickle cell painful crisis results from a centrally
mediated reflex shunting of blood away
from the bone marrow. Using a drug that could block this vasomotor reflex we
expected to prevent the painful crisis.
Material
and Methods: A prospective study using
an angiotensine- converting enzyme inhibitor (sodium fosinopril ,5 mg once a
day) in 21 patients with sickle cell disease who had severe and recurrent
painful crisis. The mean age of the patients was 27 ± 13 years. The numbers of
emergence room admissions because of painful crisis were registered 12 months
before and 13 ± 6 months after beginning the medication.. It was also studied
the hemoglobin levels, the bilirrubin unconjugated seric concentrations,the
fetal hemoglobin levels and the lactic dehydrogenase concentrations after and
before treatment.
Results:There
were no statistically difference between hemoglobim, unconjugated bilirrubin,fetal
hemoglobin and lactic dehydrogenase before and after the treatment (p=NS). The
numbers of emergence room admissions were reduced from 6,6 ± 2,3 to 1,4 ± 1,4
after the treatment (p< 0,0001).
Conclusion:
We concluded that the use of an angiotensine- converting enzyme inhibitor
prevent recurrent painful crisis in sickle cell disease.
RESUMO
Conforme uma nova hipótese, as crises álgicas da anemia falciforme resultam de um desvio reflexo do fluxo sanguíneo para fora da medula óssea, com mediação central. Usando uma droga que pudesse bloquear essa vasomotricidade reflexa, esperávamos prevenir as crises álgicas.
Material e Métodos: Foi realizado um estudo prospectivo utilizando um inibidor da enzima conversora da angiotensina (fosinopril sódico- 5 mg ao dia, via oral)em 21 pacientes portadores de anemia falciforme que apresentavam crises álgicas graves de repetição. A média de idade dos pacientes foi de 27 ± 13 anos. Os números de admissões na sala de emergência foram registrados 12 meses antes e 13 ± 6 meses após o início do tratamento.
Resultados: Não houve diferença estatisticamente significativa entre os níveis de hemoglobina, bilirrubina não conjugada e desidrogenase láctica antes e após o tratamento (p=NS). O número de admissões por crises álgicas na sala de emergência foi reduzido de 6,6 ± 2,3 para 1,4 ± 1,4 após o tratamento (p< 0,0001).
Conclusão: O uso de inibidor da enzima conversora de angiotensina preveniu a recidiva das crises álgica em pacientes com anemia falciforme.
INTRODUÇÃO
A crise álgida é uma das manifestações mais importantes da anemia falciforme. Em geral, a base patológica da crise é a necrose da medula óssea. 2,3 Tradicionalmente, o fenômeno da oclusão vascular é visto como o mecanismo fisiopatológico responsável pela crise, precipitado pelo frio, infecções, gravidez, acidose, altitude e stress. Entretanto, de acordo com Serjeant,5 a crise dolorosa é o resultado do desvio reflexo do sangue (com mediação central) para fora da medula óssea, que se torna isquêmica e, posteriormente, necrótica.
Supondo ser verdadeira esta hipótese, o uso de uma droga que bloqueasse essa vasomotricidade reflexa poderia prevenir a crise dolorosa em muitos pacientes com anemia falciforme. Iniciamos então um protocolo em que um inibidor da enzima conversora da angiotensina foi utilizado em 21 pacientes portadores de anemia falciforme com crises álgidas de repetição.
PACIENTES E MÉTODOS
Vinte e um pacientes com anemia falciforme com crises álgidas severas e recorrentes ( mais de duas crises álgidas necessitando internação num período de seis meses), requerendo admissões freqüentes na Unidade de Emergência do Instituto Estadual de Hematologia Arthur de Siqueira Cavalcanti, Rio de Janeiro,
RESULTADOS:
Um paciente foi retirado do estudo por desenvolver hipotensão sintomática após a primeira dose da medicação.
Não houve diferença estatisticamente significativa entre os estudos laboratoriais hematológicos realizados antes e após o tratamento (Tabela).
Houve uma redução significativa do número de admissões em caráter de urgência devida às crises álgicas após o início do tratamento com fosinopril sódico (6,6 ± 2,3 antes e 1,4 ± 1,4 após o tratamento, p< 0,0001).

DISCUSSÃO E CONCLUSÃO
Alguns autores (4) acreditam que as fases iniciais dos eventos vasos-oclusivos da anemia falciforme envolvem vaso-espasmo arteriolar inicial. Esta visão é baseada em estudos anatomopatológicos de pacientes com anemia falciforme, nos quais havia evidência de infarto orgânico na ausência de oclusão vascular(2) e pelo vasospasmo arteriolar retiniano visto em pacientes falcêmicos antes e após o uso de bloqueador de canais do cálcio(4). Dando maior ênfase a essa hipótese, Baum et AL (1) estudando pacientes com anemia falciforme em crises álgicas na Jamaica, notaram que em mais de metade dos casos essas crises eram bilaterais e simétricas. As crises eram precipitadas pelo frio (que inicia alterações vasculares reflexas) e sem evidências de alterações hematológicas reprodutíveis precedendo as crises. Foi levantada, então, a possibilidade de um desvio reflexo do sangue, com mediação central para fora da medula óssea, Síndrome de Steal(5). Esta hipótese aventa a possibilidade de que a medula óssea, temporariamente isquêmica, precipitaria o afoiçamento, avascularização irreversível e necrose. O processo inflamatório desencadeado pela medula óssea necrótica pode aumentar a pressão intramedular, prejudicando o fluxo sanguíneo para a medula óssea normal adjacente e promovendo novas áreas de necrose. Além disso, a medula óssea estaria susceptível ao bloqueio temporário do fluxo sanguíneo devido ao aumento da demanda metabólica e de consumo de oxigênio induzida pelo efeito secundário da hemoglobina S na hemólise e conseqüente expansão eritropoiética (5).
Nosso estudo demonstrou que a utilização de um inibidor da enzima conversora da angiotensina, fosinopril sódico, pode prevenir a crise álgica na anemia falciforme, dando maior sustentação ao papel do mecanismo neuro- humoral na gênese dessas crises. Além disso, não houve nenhuma redução nas taxas de hemoglobina,bilirrubina ou desidrogenase láctica assim como dos níveis de hemoglobina fetal.
Limitações do estudo: a principal limitação é que este não foi um estudo duplo cego e não sabemos ainda por quanto tempo se estende a duração do efeito benéfico do medicamento.
DISCUSSION AND CONCLUSION
(Some
authors (4) believe that the initial phases of the vases-occlusive events of
the sickle cell anemia involve arteriolar initial vase-spasms. This vision is
based on pathologic studies of patients with sickle cell disease, in which it
had organic evidence of infarct in the absence of vascular occlusion (2) and
for vasospasm to arteriolar retinal in sickle
cell patients before and after the use of inhibitor of calcium canals (4). Giving bigger emphasis to this
hypothesis, Baum patient et AL (1) studying with sickle cell anemia patients in painful crises in Jamaica, had noticed that
in more than half of the cases these crises were bilateral and symmetrical. The
crises were precipitated by the cold (that it initiates reflected vascular
alterations) and without evidences of reproducible hematolologics alterations
preceding the crises. It was raised, then, the possibility of a reflected
shunting line of the blood, with central mediation for is of the bone marrow,
Syndrome of Steal (5). This raises the possibility that the bone marrow, temporarily
ischemic, would precipitate the sickle process, irreversible ischemia and
necrosis. The inflammatory process unchained by the necrotic bone marrow can
increase the pressure intramedularly, harming the sanguineous flow for the
adjacent normal bone marrow and promoting new areas of necrosis. Moreover, the bone
marrow would be susceptive to the temporary blockade of the sanguineous flow
which had to the increase of the metabolic demand and induced consumption of
oxygen for the secondary effect of the hemoglobin S in hemolysis and
consequence erythropoietic expansion (5). Our study it demonstrated that the
use of an inhibitor of the converting enzyme of the angiotensina, fosinopril,
can prevent the painful crisis in the sickle cell anemia, giving to bigger
sustentation to the paper of the neuro-humoral mechanism in the genesis of these crises. Moreover, it
did not have no reduction in the hemoglobin taxes, lactic desidrogenase or
bilirrubin as well as of the levels of fetal hemoglobin. Limitations of the
study: the main limitation is that this was not a blind double study and still
does not know for how much time if it extends the duration of the beneficial
effect of the medicine.
